It is an estimate, not a laboratory measurement — mixed dishes, stews and anything layered or hidden under sauce are the hardest cases. You can always correct the portion or the ingredients before you save, and a corrected estimate is still far faster than searching a database.
No. Estimating the portion from the photo is the whole point. If you want to be precise on a particular meal you can enter the weight by hand, but nothing requires it.
